What happens to information you submit through this website, in plain terms.
This policy covers the AccessMind Psychiatry website only. Headway operates its own systems for scheduling, insurance, billing and clinical records. Information submitted directly to Headway is governed by Headway’s privacy policy.
What this site collects
If you complete the inquiry form, we receive what you enter: your name, contact details, state, what you are interested in, your contact preference and any message you write. We also receive basic technical information such as pages visited and the source that referred you, used to understand how the site performs.
The form asks you not to include medical details, symptoms, diagnoses or member ID numbers. It is a general inquiry form and is not a secure clinical channel.
If you complete the free stress and well-being check-in, your answers are used to calculate an educational indicator. AccessMind receives your overall score, result category, selected care goal, state, contact details, consent choices and website attribution. Individual question responses are not stored in the customer relationship management system.
The check-in is not a diagnostic assessment, does not provide medical advice and does not create a provider-patient relationship.
How it is used
To respond to your question, deliver your check-in summary, follow up about starting care, and send educational or scheduling information where you have agreed to receive it. Information is stored in our customer relationship management system for these purposes.
Messages and consent
Email and text follow-up only happens where you have opted in. You can stop at any time by replying STOP to a text or using the unsubscribe link in an email. Message and data rates may apply.
Sharing
Information submitted here is not sold. It is shared only with the service providers needed to operate the site and our follow-up systems, or where required by law.
Analytics
This site may use standard web analytics and advertising measurement tools to understand traffic and campaign performance. These tools collect usage data and are not used to collect health information.
Your choices
